Drug

Adenosine

6 mg IV bolus followed by 12 mg IV. Follow with 30 cc saline flush.

Amiodarone

Pulseless V-tach/V-fib: 300 mg IV push, may repeat with 150 mg
Perfusing rhythm: 150 mg IV slowly

Atropine

0.5-1 mg IV total of 0.04 mg/kg

Diltiazem

0.25 mg/kg IV over 2 minutes followed by 0.35 mg/kg. Drip at 5-15 mg/hr. See note under verapamil.

Epinephrine

1 mg IV push q 3-5 minutes (maximum of 0.02 mg/kg but high dose is of questionable benefit and may increase mortality).

Isoproterenol

2-8 mg/min for overdrive pacing of torsades de pointes.

Lidocaine

Pulseless V-tach/V-fib: 1.5 mg/kg IV push, repeat in 3-5 minutes to toal of 3 mg/kg.
Perfusing Rhythm: 1-1.5 mg/kg slowly and then 0.5-0.75 mg/kg IV in 5-10 minutes to toal of 3 mg/kg.
Drip: 2-4 mg/min.

Magnesium

1-2 g IV push over 5 min if patient has perfusing rhythm.

Metoprolol

5mg IV slowly with 5 mg every 5 minutes to total of 15 mg. Can titrate to effect.

Procainamide

17 mg/kg IV, maximum rate of 30 mg/min. Drip 1-4 mg/min.

Sodium Bicarbonate

1 mEq/kg

Sotalol

1.0 to 1.5 mg/kg infused at 10 mg/min. Do not use if poor perfusion. Negative inotrope.

Vasopressin

40 U IV single dose only

Verapamil

2.5-5 mg IV and repeat with 5-10 mg. Maximum 20 mg. Note: Can pretreat with 3.3 cc of CaCL to mitigate hypotension. Drip at 5-10 mg/hr.
